Thank you for your interest in becoming an approved Constructeam contractor. The following information must be submitted in full to begin the approval process.

• New Contractor Information Form
• Contractor Code of Ethics Agreement
• Payment Policy Agreement
• Direct Deposit Enrollment Form
• Voided Check
• Insurance Requirements (See other pages for details, minimum coverage amounts, & Sample Insurance Certificate)
• Certificate of your General Liability Insurance, with Constructeam named as Additional Insured ($1,000,000.00 minimum liability amount)
• Certificate of Your Workers Comp. Coverage ($500,000 min. amount)
• or Exclusion Form WC-337
• Independent Contractor Statement
• Standard Indemnification Agreement
• Violent Crime Control & Background Consent Form
• W-9 Form
• Subcontractor Warranty Form
• Waiver of Lien & Warranty Paperwork
• Lead Renovator’s Certificate (if applicable)

Please have your insurance certificates, with Constructeam added as an Additional Insured, sent directly to us by your insurance agent, via e-mail to accounts@restoreitright.com
